Community-Hospital Nutrition Education Program

Grant Guidelines
In order to
receive a grant to fund your program, your program MUST adhere to the following guidelines. Should you have any questions, please contact
us at 215-389-2300. Together we can
ensure that your program qualifies to receive our grant.
1. 10 minute heart-health presentation by a cardiologist or other appropriate
health professional who is qualified to answer questions from the
audience.
2. 10 minute nutrition presentation by a nutritionist, dietician, or
other appropriate health professional who is qualified to answer questions from
the audience.
3. Heart healthy cooking demonstration and samples for the participants.
4. Provide the recipe(s) to the participants.
5. Sign-in registration sheet of all participants who
attend the program. In order to be
reimbursed for each person, they must remain through the duration of the
program.
6. Four (4) clear digital photos from your session. If you do not have a digital camera, four (4)
clear pictures (NO POLAROIDS) will suffice.
Send within one (1) week of session.
7. CVI logo on ALL print and promotional materials as a program
co-presenter and sponsor.
8. If possible, run a post-event story in your hospital/community
newsletter.
9. Completion and submission of a post-event coordinator
evaluation that the CVI will supply. Send within 2 weeks of session.
10. Evaluation forms to be filled out by audience
members.
11. Minimum of 25 audience members.
What the CVI will
provide for your Program

Nutritional
education grants for hospitals and community organization to be reimbursed to
the hospital/organization for program expenses up to $10 per person.
·
Logistical
support in planning and implementing your program.
·
Educational
materials.
·
A
cookbook to raffle at your program.
·
Aprons
for your speakers.
·
Evaluation
form for your audience to fill out after the program.
·
Media
resources: including pre and post event
press releases.
How your Grant will be
distributed
·
The
amount of each grant for reimbursement of program expenses is a maximum of $10
per person attending the program. There
is a minimum requirement of 25 audience members with a maximum attendance of
150 for grant purposes.

You
may have more than 150 people attend, but the maximum grant that can be issued
is $1,500.
·

refer to the program’s application budget and grant worksheet. This will give you an idea of what to expect
in terms of total grant amount.
